Immunization of Pupils in School

DPT A minimum of 4 doses of DPT are required. One dose must have been administered on or after the fourth birthday. Any 5 doses will complete the requirement.

POLIO A minimum of 3 doses of oral polio vaccine are required. One dose must be given on or after the fourth birthday. Any 4 doses will complete the requirement.

MEASLES Pupils who present documented laboratory evidence of measles immunity shall not be required to receive the vaccine. Two doses of live measles-containing vaccine are required. The first dose must be given on or after the first birthday, and the second dose shall be administered no less than one month after the first dose. All children born on or after January 1, 1990 must meet this requirement.

MUMPS/ RUBELLA One dose of mumps and rubella vaccine must be administered on or after the first birthday. Pupils who present documented laboratory evidence of mumps/rubella immunity shall not be required to receive the vaccines.

HEPATITIS
1. Every child born on or after January 1, 1996 and entering kindergarten or first grade shall have received three doses of HBV.

2. Every child born on or after January 1 1990 and entering 6th grade shall have received 3 doses of HBV or 2 doses of Merck’s Recombivax HB. (Recombivax applies to children 11-15 years of age only)

VARICELLA Every child born on or after January 1, 1998 and/or entering school will be required to have received one dose of the varicella (chicken pox) vaccine.
