IMMUNIZATION INFORMATION:

The New Jersey Department of Health (NJDOH) identifies which immunizations are required for school attendance. Students who have not submitted proof of state mandated immunization requirements will be excluded from school until ALL requirements have been met.

Preschool Immunization Requirements

• Preschool students are also required to have a yearly flu shot up until the age of five, as well as a vaccine of PCV and HIB on or after the first birthday.

K-8 Requirements

• Grade 7: Students above ten (10) years of age and in 7th grade must complete the 7th grade immunization requirements.

PHYSICAL EXAMINATION

A current physical exam is required for school attendance.  Proof of this exam must be submitted within 30 days of the first day of school.  This exam must have been performed within the past 365 days of your child’s first day of school. 

Special Note: Students entering Galloway Township Public Schools from out of State or Country have 30 days from the first date of attendance to produce documentation of required IMMUNIZATIONS/PHYSICAL.
